About the Role:

This exciting new role provides a high standard of triage and referral allocation, for the AAH Clinical Care Team and external partners.

Reporting to the Clinical Care Manager, you will ensure the effective coordination of client assessments, monitoring clients clinical needs and goals, providing consistent service delivery and follow up of direct and indirect care.

You will contribute to a culture of customer excellence, by providing holistic services and assisting your team to deliver services to our clients and their families. Fostering positive working relationships, you will ensure our valued clients physical health is maintained at the highest possible level.

Skills, knowledge, attributes and experience required:

*Current AHPRA registration - Enrolled Nurse (Division 2)

*General Registration (be able to administer medications  within the skill set and enrolled nurses board guidelines) is essential

*Min 3 years’ experience in Aged Care, preferably in-Home Care Setting

*Strong interpersonal, negotiation and problem-solving skills

*Very good organisational skills;

*The ability to communicate effectively with staff and management and work as part of a team;

*A commitment to the Christian values of Anglicare;

*Excellent clinical knowledge; and

*Demonstrated experience working in a multi-disciplinary team

This role will ideally suit an experienced aged care, Enrolled Nurse who has a passion for providing holistic clinical care and is an active member of continuous improvement clinical practices.
