Anglicare currently has some fantastic roles for Case Managers who thrive on building relationships and supporting older people to live their best lives to join our newest, diverse, exciting care delivery team based in the Blacktown Region!
Based in the Blacktown Region, the Case Managers will provide onboarding and case management support for residents living independently in Anglicare Communities and in the local Blacktown area. They will ensure that residents/clients receive proactive case management, are cared for and retained by regular assessment of needs and partnering with them to ensure they fully maximise all the funding that is available to them while proactively advocating for them to ensure they receive the right level of services and support as their needs change.
About the role:
To be successful in this role you will need to have:
- Previous experience in the aged care sector, with experience in undertaking comprehensive assessments in a home setting.
- Previous experience in delivering exceptional customer service.
- Proven track record of meeting KPI’s
- Have a comprehensive knowledge and experience working in Aged Care funded programs, Home Care Packages, CHSP & STRC.
- A forward thinking attitude to establishing and contributing to the success of innovative programs and service delivery models.
- A strong passion to support older people live their best lives
